Male and Female Hormone Replacement Therapy

Dr. Biskup has received additional training and certification in Optimization of Male and Female Hormone Replacement Therapy.   Dr. Biskup, Carmen Magistro, PA-C, and Emily Davie, PA-C also have training and certification in Hormone Pellet implantation. 

Hormone Pellet implantation is not filed with insurance as it is not a coverable expense.  To keep our pricing reasonable, pellets are self-pay at time of service and payment should be made to Hormone and Lifestyle Interventions of NE.

Robert Saniuk, M.D.  Retirement

Dr. Saniuk has retired from practicing medicine.  We have other providers who will be able to continue your care at Bellevue Family Practice. 

Flu Vaccinations

We do offer the regular dose and also the high dose for patients 65 and older.  Please call in advance to schedule a time for your injection.  

COVID19  Vaccine

We do not office Covid 19 vaccinations or boosters.  Please check with your pharmacy.  

COVID19  Testing

We offer Covid testing for current and new patients 12 and older by appointment.   Please call 402/ 292-9170 to schedule a test. We do not provide covid vaccine exemption letters.  Charges will be billed to insurance and if you have any out of pocket due after your plan processes the claim, we will send you a bill.  Rapid flu test are processed at our office with results in 20 minutes.  Covid19 PCR tests will be sent to an outside laboratory – results can take 24-48 hours.  Any cost for this test will be billed by the testing lab.  We will bill for the collection of the specimen.    Please call your insurance company if you have questions regarding coverage for this.   

You will be expected to present your current photo ID and insurance card if we do not already have on file.  A staff member will come out to the hallway and get the card(s) to copy or you can email the front and back of your insurance card prior to your test if we do not have a current card on file.  You may email a copy of the front and back of your insurance card and ID to [email protected] or [email protected]   If you do not have the capability to do this, please let billing know at 402/292-9170, option 3.

Self-pay patients will be expected to pay at time of the appointment.  You can call the billing department at 402/292-9170, option 3 to pay by phone. 

Appointments

Same day/next appointments are available and we will do our best to get you in as soon as possible. Walk-ins are welcome and we will try to schedule you for a same day appointment if we have availability, however, we prefer that you call in advance to schedule.   If your symptoms are urgent and we are unable to get you in that day, we ask that you go to the Urgent Care or the nearest Emergency Room.  

Paperwork

You may be asked to update your patient information form, health history, HIPPA and insurance paperwork as current information is required as an office policy as well as by your insurance company.  We will also verify your insurance prior to appointments so we can minimize your time spent in the reception area.  We want to make sure we have accurate information to submit your claims to your insurance company in a timely manner.  We will do our best to ensure your insurance is in network, however, with over 2,000 insurance companies and plans, it is ultimately your responsibility to know your own insurance plan.  Our claims are billed as a group,- Bellevue Family Practice with tax ID, 47-0782740.  This is the information you need to provide your insurance company when checking if you are able to be seen at our office.   If your plan is associated with CHI, we are not in network.   Photo identification is required for your protection in case of identity theft as well as identifying who we are treating. We will not be able to treat you without a photo identification – please bring your current ID to your appointment as well as any applicable co-payments.  

Patient forms are available on the bottom of the website if you would like to print the forms in advance and complete them prior to your appointment. 

Patient Portal

Effective April 2020, your statements will be available online.   Some medical records may also be able to be viewed online.   If you do not already have a log in, you may request a user ID by clicking on  Patient Portal Medi-Touch on the menu or call our office at, 402 292-9170 option 3.

Our History

Bellevue Family Practice was founded in 1983 by Dr. Robert Saniuk. He partnered with Dr James Biskup in 1991. Both of the owners have strong roots in this area, and thoroughly enjoy practicing in Bellevue where they can remain close to family and friends. They both maintain personal relationships with many of their patients. Dr. Saniuk has retired from practicing medicine and Bellevue Family Practice is now owned by Dr. James Biskup.

Bellevue Family Practice has grown over the years from a relatively small office and patient base to a large independently owned practice that is served by one physician and four physicians assistants with over 120 years of combined experience, caring for a large diverse patient base. We offer many services right here in the clinic to make it as convenient for our patients to obtain the needed services.
